Kit for determination of alpha-1 microglobulin (A1m) on routine biochemical analyzers. The kit includes a reagent, a calibrator and a kit. Under normal conditions, similar to other microproteins, A1m is almost completely reabsorbed by the renal tubules and only a very small amount is excreted in the urine. An increase in urine concentration is therefore indicative of proximal tubule lesions and/or tubular dysfunction. An increased excretion rate may indicate tubular damage associated with renal tubulointerstitial nephritis or tubular toxicity due to exposure to nephrotoxic drugs or heavy metals. Glomerulonephropathy and renal vasculopathy are also often associated with coexisting tubular damage and thus may lead to increased excretion. An increased value of A1m in patients with urinary tract infections may indicate kidney involvement (pyelonephritis). A1m is considered a good marker of tubular dysfunction because of its relatively constant rate of production and stability in urine, even at altered pH. A1m can be used as a diagnostic marker of preeclampsia. Oxidative stress in placental tissue triggers the endogenous synthesis and plasma concentration of this protein. Applications for commonly used biochemical analyzers available (Advia®, Alinity C, AU®, Atellica™, cobas® C, Optilite®, Mindray and others).
